Sociaal psychiatrisch verpleegkundige bij Gebiedsteam Maarssen Wil jij als sociaal psychiatrisch verpleegkundige werken in de specialistische GGZ en echt iets betekenen voor mensen met ernstige psychiatrische aandoeningen (EPA)? Bij Gebiedsteam Maarssen krijg je veel zelfstandigheid en ruimte om jouw SPV-expertise in te zetten. Je komt bij cliënten thuis, werkt nauw samen met een breed multidisciplinair team en hebt een afwisselende functie waarin geen dag hetzelfde is. Wat ga je doen? Gebiedsteam Maarssen is onderdeel van Altrecht en biedt specialistische GGZ aan volwassenen in Stichtse Vecht en De Ronde Venen. We werken volgens de F-ACT-methodiek en bieden onze zorg ambulant, outreachend en zoveel mogelijk in de eigen omgeving van de cliënt. Voor huisbezoeken en andere afspraken staan drie leaseauto's voor het team klaar. Als SPV heb je een eigen caseload en werk je met volwassenen met ernstige psychiatrische aandoeningen (EPA). Je voert intakes uit, stelt behandelplannen op en biedt herstelgerichte, systeem- en klachtgerichte behandeling. Daarbij heb je aandacht voor verschillende levensgebieden en werk je samen met de cliënt aan passende doelen. Ook speel je een belangrijke rol bij crisissituaties en bied je bemoeizorg wanneer contact of behandeling niet vanzelfsprekend tot stand komt. Je werkt nauw samen met psychiaters, verpleegkundig specialisten, psychologen, GGZ-agogen, ervaringsdeskundigen en ketenpartners zoals huisartsen en woonbegeleiding. Je schakelt tussen cliëntcontact, overleg en onverwachte situaties. Daarnaast draai je enkele keren per maand mee in de 24-uurs bereikbaarheids- en aanwezigheidsdiensten van de crisisdienst van Altrecht. Hiervoor ontvang je passende toeslagen en compensatie.
