Jouw rol als sociaal psychiatrisch verpleegkundige Als SPV bij Prisma werk je op het snijvlak van zorg voor mensen met een verstandelijke beperking en de psychiatrie. Je bent een onmisbare adviseur en behandelaar voor cliënten met een verstandelijke beperking en bijkomende psychiatrische problematiek (dubbel- en triple problematiek). Je kijkt verder dan het gedrag; je zoekt naar het verband, waarin de cliënt centraal staat met oog voor het systeem. Wat ga je doen? Als SPV houd je je bezig met complexe psychosociale, psychiatrische en systemische problematiek voor onze doelgroep: mensen met een verstandelijke beperking. Je doet zowel zelfstandige behandeling, maar maakt ook deel uit van het multidisciplinaire team. Je hebt hierbij een coördinerende rol. Een SPV binnen Prisma wordt onder andere ingezet voor: - Consultatieve en structurele betrokkenheid. - Betrokkenheid bij crisis voorziening (screening en opvolging). - Casemanagement. - Het adviseren van begeleidende teams en hun netwerk bij complexe casuïstiek en crisissituaties. - Het onderhouden van nauwe contacten met het netwerk van de cliënten en externe ketenpartners (zoals de GGZ, verslavingszorg, politie, gemeente etc). - Het vertalen van behandelplannen naar praktische handvatten voor de werkvloer. - Het verzorgen van scholingen over onder andere suïcidepreventie.
