Dan is deze functie zeker iets voor jou! Als sociaal psychiatrisch verpleegkundige / ambulant verpleegkundige bij Team Vangnet werk je in de wijk met Amsterdammers die extra ondersteuning nodig hebben. Je bezoekt cliënten thuis, reageert op meldingen van woningcorporaties, politie of buurtbewoners en beoordeelt welke zorg passend is. Soms gaat het om complexe problematiek of acute situaties, waarbij je directe zorg biedt of specialistische hulp inschakelt. Je wordt ingezet bij situaties waarin een sociaal psychiatrisch verpleegkundige (SPV) nodig is, zoals aanhoudende overlast of complexe problematiek. In deze crisissituaties is afstemming met collega's en ketenpartners essentieel. Dit doe je onder andere door: - op meldingen af te gaan met bijvoorbeeld de wijkagent, woningcorporatie of buurtbewoners en de situatie ter plaatse te beoordelen; - acute zorg te bieden bij verzoeken van politie of maatschappelijke opvangorganisaties en, indien nodig, cliënten door te verwijzen naar specialistische of aanvullende zorg; - deel te nemen aan multidisciplinaire overleggen met ketenpartners en de zorg af te stemmen.
