Je nieuwe functie Als sociaal psychiatrisch verpleegkundige bij Van Andel Ouderenpsychiatrie in Drachten ben je nauw betrokken bij de behandeling van oudere en vaak kwetsbare patiënten met uiteenlopende psychiatrische problematiek. Vanaf de start van het behandeltraject onderzoek je samen met de patiënt wat voor hem of haar van waarde is in het dagelijks leven. Daarbij staan perspectief, vertrouwen en eigen regie centraal. Je ondersteunt patiënten bij het omgaan met blijvende kwetsbaarheden en helpt hen om, ondanks de uitdagingen die zij ervaren, een zo betekenisvol en zelfstandig mogelijk leven te leiden. Je maakt onderdeel uit van een multidisciplinair team en neemt actief deel aan gezamenlijke overleggen. Daarnaast vervul je een belangrijke rol in het aanmeldproces. Je voert en coördineert intakes in samenwerking met de regiebehandelaren en zet jouw expertise in bij complexe behandeltrajecten van patiënten met multiproblematiek, zowel op de poli als in de thuissituatie. Ook werk je nauw samen met verschillende ketenpartners, zoals sociale wijkteams, thuiszorgorganisaties en huisartsen. Daarmee draag je bij aan een integrale aanpak die aansluit bij de behoeften van de patiënt. Wat staat je te wachten - Afwisseling in je werk, want jij hebt een eigen regierol in de triade - Werken met de ouderendoelgroep, waarbij sprake is van psychiatrische problematiek veelal in samenhang met somatische klachten en beperkende psychosociale omstandigheden - Veel ruimte, verantwoordelijkheid en de kans om te doen waar je echt goed in bent - Arbeidsvoorwaarden conform CAO GGZ, inschaling in FWG 55, min. € 4.000,- max. € 5.294,- o.b.v. fulltime dienstverband - Balansbudget van € 1.000, - per jaar bij een voltijd dienstverband - Persoonlijk opleidingsbudget, t.b.v. accreditatie, van € 2.000, - per jaar - Een arbeidsovereenkomst voor een jaar, met uitzicht op een vast dienstverband - Volop mogelijkheden om specifieke deskundigheid te ontwikkelen - Wer...
