Wil je deel uitmaken van een hecht en professioneel FACT-team waar samenwerking, herstelgericht werken en oog voor zowel cliënten als collega's centraal staan? Dan is dit jouw kans! Takenpakket Als SPV binnen ons FACT-team in Tiel ben jij een belangrijke schakel in de zorg voor onze cliënten. Je hebt een eigen caseload en werkt nauw samen met collega's zoals verpleegkundigen, maatschappelijk werkers en regiebehandelaren om de juiste zorg op maat te bieden. Je werkzaamheden omvatten onder andere: - Opstellen en uitvoeren van behandelplannen in samenwerking met de regiebehandelaar; - Begeleiden en ondersteunen van cliënten met een voornamelijk psychotische kwetsbaarheid, vaak gecombineerd met problematiek zoals verslaving, LVB of bipolaire stemmingsstoornis; - Opbouwen en onderhouden van een sterk netwerk rondom de cliënt, met o.a. ketenpartners (zoals bijvoorbeeld RIBW en Iriszorg) en naasten; - Werken met een shared caseload, zodat het team gezamenlijk de verantwoordelijkheid draagt; - Aan de slag met innovatieve methoden zoals de Netwerk Intake voor het betrekken van de directe omgeving; - Omgaan met zorgmijders en gedwongen zorg, waarbij je een balans zoekt tussen vrijheid en veiligheid; - Eén keer per week een bureaudienst draaien als aanspreekpunt voor externe contacten. Op die dag hou je ruimte in de agenda om te kunnen inspelen op crisissituaties; - Meedraaien van diensten bij de crisisdienst, naast je reguliere uren, als de organisatie dat noodzakelijk acht.
