Wat ga je doen Je taken en verantwoordelijkheden zijn gevarieerd en veelomvattend: * Je verleent directe hulpverlening aan cliënten met ernstige, complexe psychiatrische problematiek en hun systemen. * Je werkt nauw samen met collega's in het multidisciplinaire team. * Je stelt sociaal psychiatrische diagnoses op basis van klinisch redeneren. * Je coördineert het behandelproces van de cliënt, je bent aanspreekpunt voor de betrokkenen en je stelt de benodigde plannen op (behandelplan, risicotaxatie en andere deelplannen). * Je signaleert het effect van de ingezette behandeling en eventuele nieuwe aandachtspunten. * Je levert met enthousiasme een bijdrage aan de kwaliteit van de zorg en het verder vormgeven van de ouderen ggz. * Je maakt gebruik van intensieve sociaal psychiatrische, begeleiding- en psychotherapeutische methodieken vanuit diverse referentiekaders. * Emergis heeft een 24-uurs crisisdienst buiten kantooruren. Je draait mee in deze diensten. Samen aan de slag Mentale gezondheidsproblemen kunnen ieder van ons treffen.
