Waar kom je te werken? Op de afdeling Ambulante Behandeling Ouderen Zwolle richten we ons op de groep Ouderen met ernstige psychiatrische stoornissen. Wij bieden onze patiënten een uitgebreid aanbod van diagnostiek en behandeling, waarbij deskundigheid, betrokkenheid en samenwerking voorop staat. Wij willen patiënten zo goed mogelijk behandelen wanneer er bij hen sprake is van psychiatrische problemen in de derde en vierde levensfase en, zoveel als mogelijk, in hun eigen omgeving. Wat ga je doen? Als Sociaal Psychiatrisch Verpleegkundige verricht je diagnostiek, intakes, behandeling en begeleiding van patiënten en hun naasten. Je stelt samen met collega's behandelplannen op en voert deze uit. Je voert gesprekken met patiënten op kantoor en in de thuissituatie en levert een bijdrage aan groepsbehandelingen. Daarnaast coördineer je het zorgproces, zorg je voor een goede verslaglegging en intervenieer je waar nodig in crisissituaties. Je werkt in een multidisciplinaire ambulante setting, waarin je nauw samenwerkt met collega's en ketenpartners rondom de patiënt. Je neemt deel aan zorgafstemmingsgesprekken (ZAG), multidisciplinaire overleggen en team- en beleidsvergaderingen.
