Inhoud De afdeling Sociaal Wijkteam (SWT) wordt gevormd door vijf wijkteams, een backoffice, een team Leerplicht/Doorstroompunt/ Relatie- en scheidingsteam en een team materiele wmo. Deze teams voeren de Jeugdwet, Wet Maatschappelijke ondersteuning en de Participatiewet uit. De wijkteams vormen een laagdrempelige plek in de wijk voor bewoners met vragen over zorg of ondersteuning. Vanuit een integrale aanpak kijken we wat nodig is, maar ook wat bewoners zelf kunnen doen. We werken intensief samen aan onze opgaven met (keten)partners in de stad, zoals onderwijs, zorgaanbieders, wijkverpleegkundigen, wijkagenten en vrijwilligers- en bewonersorganisaties. Opdracht Als sociaal werker ben je nauw betrokken bij inwoners van Zwolle die hulp nodig hebben. Je handelt meldingen af en helpt mensen met uiteenlopende vraagstukken rond.
