Functieomschrijving Binnen het team WMO van gemeente Achtkarspelen zoeken wij op korte termijn een Sociaal Werker WMO voor 36 uur per week. Als Sociaal Werker (specialisme WMO) voer je onderzoeken uit volgens de vijfstappenmethodiek en onderbouw je je bevindingen binnen het geldende juridische kader. Op basis van je analyse geef je in het gespreksverslag een zwaarwegend advies, waarbij je rekening houdt met voorliggende voorzieningen, eigen kracht en het netwerk van de inwoner. De functie is ingeschaald in schaal 9 van de cao Gemeenten en wordt uitgevoerd vanuit de locatie in Buitenpost. Gevraagd profiel - HBO-opleiding in Sociaal Werk, Maatschappelijk Werk of een vergelijkbare richting. - Aantoonbare ervaring en deskundigheid op het gebied van WMO-indicatiestelling en de vijfstappenmethodiek. - Goede kennis van het juridisch kader rondom de WMO en de vaardigheid om daarin onderbouwde adviezen te formuleren. - Sterke communicatieve vaardigheden, gespreksvaardig in het opstellen van heldere gespreksverslagen en adviezen. - Werken vanuit eigen kracht en netwerkgericht denken bij cliënten, met aandacht voor voorliggende voorzieningen. - Beschikbaarheid voor 36 uur per week en inzetbaar vanuit de locatie in Buitenpost / gemeente Achtkarspelen. - Alleen kandidaten die aantoonbaar in loondienst zijn bij de inschrijvende partij komen in aanmerking; bereidheid tot het overleggen van bewijsmateriaal over de arbeidsrelatie is vereist.
