Sociaal wijkteamlid WMO Voorzieningen Venlo Over de functie JOB-3602056 Je gaat aan de slag als sociaal wijkteamlid WMO Voorzieningen binnen de sociale wijkteams. In deze rol ondersteun je inwoners bij hun hulpvragen en onderzoek je samen met hen welke ondersteuning nodig is om zo zelfstandig mogelijk te kunnen leven. Je kijkt verder dan de eerste vraag en brengt de bredere ondersteuningsbehoefte zorgvuldig in beeld. Daarbij werk je nauw samen met inwoners, hun netwerk en ketenpartners om passende oplossingen en doelen te realiseren. Wat ga je doen - Je voert keukentafelgesprekken met inwoners en hun netwerk of mantelzorgers. - Je stelt in afstemming met de inwoner een leef-zorgplan op, gericht op het bereiken van concrete doelen. - Je stimuleert actief het inzetten van eigen kracht en mogelijkheden van inwoners. - Je volgt de voortgang van de ondersteuning en toetst of de doelen uit het leef-zorgplan worden bereikt. - Je signaleert of de geboden ondersteuning helpt bij het oplossen van de ondersteuningsvraag. - Je stelt de inwoner centraal en betrekt waar nodig ketenpartners bij de uitvoering van het leef-zorgplan. - Je levert een bijdrage aan een team dat steeds kijkt hoe het werk anders, beter en slimmer kan. Over jou - Je bent betrokken bij inwoners en hebt oog voor de vraag achter de vraag. - Je staat stevig in je schoenen en weet goed om te gaan met complexe en dynamische situaties. - Je werkt zelfstandig, maar zoekt ook actief de samenwerking op binnen het team en met partners. - Je bent gericht op het vinden van passende oplossingen binnen de mogelijkheden van het sociaal domein. Eisen - Je hebt een afgeronde relevante hbo-opleiding. - Je hebt aantoonbare werkervaring als Wmo-consulent voorzieningen of je hebt een ergotherapeutische achtergrond. - Ervaring als Wmo-consulent en/of gemeentelijke ervaring - Je hebt ervaring met regievoeren ten aanzien van meervoudige en/of complexe problematiek. - Je hebt goede gespreksvaa...
