In het kort: Je gaat als sociaal wijkteamlid Wmo keukentafelgesprekken voeren, leef-zorgplannen opstellen, voortgang en resultaten monitoren en samen met ketenpartners passende ondersteuning organiseren rond de hulpvragen van inwoners. Als sociaal wijkteamlid Wmo heb je de volgende taken/ verantwoordelijkheden: - Je voert keukentafelgesprekken met de inwoner (en zijn netwerk/mantelzorger) en stel je in afstemming met hen een leef-zorgplan op gericht op het bereiken van doelen. Je stimuleert actief het inzetten van eigen kracht en mogelijkheden van de inwoners. - Je bent actief betrokken bij inwoners en toets of de doelen die inhet leef-zorgplan zijn beschreven worden bereikt, je hebt hierin een signalerende rol en kijkt of de geboden ondersteuning geholpen heeft bij het oplossen van de ondersteuningsvraag. - Je bent in staat de inwoner centraal te stellen en ketenpartners te betrekken bij de uitvoering van het leef-zorgplan. Waar kom je te werken? Je thuisbasis het team Sociale Wijkteams. Het team helpt inwoners die ondersteuning nodig hebben om zo zelfstandig mogelijk te leven. Denk aan huishoudelijke hulp, jeugdhulp en hulp aan gezinnen met meerdere problemen. We kijken altijd naar de vraag achter de vraag om een zo compleet mogelijk beeld te krijgen van de ondersteuningsbehoefte: gaat het bij bijvoorbeeld een scootmobiel echt om een mobiliteitsprobleem probleem om zich te verplaatsen, of leeft de wens aan sociale contacten? We luisteren goed, vragen door en willen écht helpen. Soms zoeken we daarbij de grenzen op van wat kan en mag. We zijn een hecht team dat stevig in de schoenen staat en steeds kijkt hoe het anders, beter en slimmer kan. Een team vol dynamiek en drive! Voor deze opdracht gaat de voorkeur uit naar iemand met ervaring als Wmo-consulent en/of gemeentelijke ervaring. Gezien de huidige situatie is er helaas onvoldoende tijd en ruimte om iemand uitgebreid in te werken. Kandidaten moeten daarom bij voorkeur direct inzetbaar zijn. Daarnaast heeft ee.
