Als specialist ouderengeneeskunde maak je dit betekenisvolle verschil iedere dag opnieuw in de prachtige regio Beuningen, waar jouw deskundigheid direct bijdraagt aan een waardevoller bestaan voor ouderen. In deze rol ben je de medische spil in de eerstelijns ouderenzorg. Je stapt binnen in de leefwereld van cliënten en kijkt altijd verder dan alleen de fysieke klachten. Samen met een hecht netwerk van huisartsen, wijkverpleegkundigen en casemanagers zorg je ervoor dat ouderen zo lang mogelijk zelfstandig en waardig thuis kunnen blijven wonen. Jouw werkdag is ontzettend afwisselend en vraagt altijd om een flexibele, mensgerichte aanpak binnen een dynamische zorgomgeving. Om je een helder beeld te geven van de praktijk, omvat jouw takenpakket onder andere de volgende concrete werkzaamheden: - Het afleggen van huisbezoeken om de medische en sociale situatie van kwetsbare ouderen grondig te beoordelen. - Het nauw afstemmen van persoonlijke behandelplannen met huisartsen, wijkverpleegkundigen en andere zorgprofessionals. - Het bieden van specialistisch medisch advies dat direct bijdraagt aan de kwaliteit van leven en het welzijn van de cliënt.
