- Specialist ouderengeneeskunde in de eerste lijn 24 - 36 FWG 75 € 6.717,48 - € 10.467,78 Para (Medisch) Specialistische teams (Rijswijk) De uitdaging Door de groei van onze eerstelijns ouderenzorg breiden wij ons team uit met een specialist ouderengeneeskunde (SO). Florence speelt in de regio een actieve rol in de ontwikkeling van de eerstelijns ouderenzorg. Samen met huisartsen, ziekenhuizen en andere zorgpartners werken we aan nieuwe vormen van samenwerking en zorg voor thuiswonende kwetsbare ouderen. Als specialist ouderengeneeskunde lever je hierin een belangrijke bijdrage en denk je mee over de verdere ontwikkeling van de eerstelijns ouderenzorg. Verder draag je actief bij aan passende zorg voor thuiswonende ouderen en versterk je de samenwerking binnen de eerste lijn. Daarnaast neem je, samen met de andere artsen binnen de organisatie, deel aan de dienstenstructuur voor de intramurale locaties. Dit ga je doen - Je werkt nauw samen met huisartsen, praktijkondersteuners, wijkverpleegkundigen, casemanagers, psychologen en andere paramedici. - Je adviseert binnen bestaande GPO's bij huisartsen en ontwikkelt waardevolle samenwerkingsrelaties met andere partijen in de regio. - Je diagnosticeert, behandelt en begeleidt thuiswonende cliënten met complexe problematiek. - Je bent betrokken bij consultaties rondom acute problematiek bij thuiswonende kwetsbare ouderen. Via de spoedconsultfunctie ben je direct bereikbaar voor huisartsen en kun je zo nodig snel een huisbezoek afleggen. - Je werkt actief samen met een regionaal ziekenhuis binnen een Geriatric Emergency Medicine Team (GEM-team) op de SEH en in de kliniek, waar je enkele dagen per maand meedraait.
