Specialist Ouderengeneeskunde in de eerste lijn Artsen en specialisten Gorinchem 24 - 28 uur 31-12-2026 Ben jij een specialist ouderengeneeskunde die verder kijkt dan de muren van het verpleeghuis? Die gelooft dat de beste zorg ontstaat door samen te werken met cliënten, naasten en collega's? Dan voel jij je thuis bij Rivas Zorggroep. Wat ga je doen? Jij gelooft dat de beste zorg niet altijd binnen de muren van een verpleeghuis wordt geleverd. Als specialist ouderengeneeskunde help je kwetsbare ouderen om zo lang mogelijk zelfstandig en met kwaliteit van leven thuis te blijven wonen. Je kijkt verder dan de zorgvraag van vandaag en hebt oog voor de mens achter het dossier. Geen dag is hetzelfde. Je gaat op huisbezoek, voert geriatrische assessments uit en denkt mee over complexe vraagstukken, zoals cognitieve achteruitgang, mobiliteit, valpreventie, prognostiek en zorg rondom het levenseinde. Met jouw expertise geef je richting, bied je duidelijkheid en help je cliënten, naasten én collega's bij het maken van de juiste keuzes. Je bent een belangrijke sparringpartner voor huisartsen, praktijkondersteuners en andere zorgprofessionals. Samen met wijkverpleegkundigen, casemanagers dementie en mantelzorgers zorg je ervoor dat cliënten de juiste zorg krijgen, op de juiste plek en op het juiste moment. Je staat er daarbij nooit alleen voor. Binnen Rivas werk je samen met een enthousiast multidisciplinair behandelteam van onder andere psychologen, fysiotherapeuten, ergotherapeuten en logopedisten. Is tijdelijk revalideren of een opname toch nodig? Dan zorg jij voor een warme overdracht en een soepele overgang. Je krijgt veel vrijheid om je vak uit te oefenen, mee te denken over innovatieve zorgvormen en jezelf verder te ontwikkelen.
