En krijg je energie van complexe zorgvragen, korte lijnen en samenwerken met huisartsen en andere zorgprofessionals? Dan maken we graag kennis met je. Voor onze MESO-praktijk in Zoetermeer zoeken wij een enthousiaste Specialist Ouderengeneeskunde voor 12 uur per week. Wat is MESO? MESO staat voor Multidisciplinaire Eerstelijns Samenwerking voor Ouderen. Binnen de MESO-praktijk brengen we specialistische ouderengeneeskundige expertise naar de eerste lijn. De Specialist Ouderengeneeskunde en Geriatrisch Verpleegkundige werken nauw samen met huisartsen en andere professionals rondom kwetsbare ouderen. We kijken breed: niet alleen naar medische problemen, maar ook naar cognitie, psychisch welzijn, functioneren, de sociale omgeving en de wensen van de oudere en diens naasten. Zo bieden we passende, samenhangende zorg dichtbij huis. Jouw rol Als Specialist Ouderengeneeskunde: - verricht je specialistische diagnostiek en beoordeling bij complexe problematiek; - voer je gesprekken met ouderen en hun naasten; - verricht je lichamelijk onderzoek en beoordeel je medicatie; - stel je samen met de oudere, naasten en betrokken professionals een behandel- en zorgplan op; - werk je nauw samen met de Geriatrisch Verpleegkundige; - stem je af met professionals uit het medische, verpleegkundige en sociale domein; - draag je bij aan kennisdeling en goede samenwerking binnen het netwerk; - denk je mee over de verdere ontwikkeling van de MESO-praktijk in Zoetermeer. Je werkt in een diagnostische en behandelfase en hebt daarbij ruimte voor eigen professionele inbreng.
