Je coördineert, ondersteunt en begeleidt zowel cliënten als medewerkers binnen onze thuiszorgdiensten en draagt zo bij aan een klantgerichte, kwalitatieve zorgverlening. Je bent een bruggenbouwer tussen mensen en diensten, houdt van structuur én innovatie, en weet medewerkers te motiveren en begeleiden. Dankzij jouw leiderschap groeit het team in kracht, collegialiteit en effectiviteit. Je combineert beleidsinzicht met praktische organisatie en bent niet bang om de handen uit de mouwen te steken.

Wat ga je doen?

  • Het voeren van sociaal financieel onderzoek door middel van huisbezoeken.
  • Bepalen van zorg- en ondersteuningsdoelen.
  • Opmaken van zorg- en hulpverleningsplan.
  • Mensen informeren en gericht doorverwijzen.
  • Organiseren van multidisciplinair overleg, verslaggeving hiervan.
  • Opmaken en bijsturen van werkplanning.
  • Zorgen voor een goede samenwerking met externe partners in de dienstverlening/ netwerking.
  • Zorgen voor een heldere, klantvriendelijke communicatie die de visie van de organisatie uitdraagt en zorgt voor toegankelijke dienstverlening.
  • Leidinggeven op een mensgerichte, flexibele manier waarbij je de missie en visie van de organisatie vertaalt naar duidelijke richting voor je team.

Profiel

  • Je beschikt over een sterke affiniteit met de thuiszorg- en welzijnssector;
  • Je hebt ervaring met cliëntgerichte dienstverlening en het voeren van intake- en begeleidingsgesprekken;
  • Je kan medewerkers coachen, motiveren en constructieve feedback geven;
  • Je beschikt over organisatorische vaardigheden en kan complexe planningen opmaken en opvolgen;
  • Je werkt nauwkeurig en heeft ervaring met dossierbeheer en administratieve opvolging;
  • Je kan samenwerken met diverse interne en externe partners;
  • Je communiceert vlot, klantgericht en respectvol;
  • Je handelt oplossingsgericht en behoudt het overzicht in situaties met een hoge werkdruk.

Diplomavereisten

  • Houder zijn van een bachelordiploma, gelijkwaardig of daarmee gelijkgesteld. Bij voorkeur een diploma verbonden met de welzijnssector.
  • Kandidaten dienen daarnaast aantoonbaar verantwoordelijk geweest te zijn voor één of meerdere van onderstaande taken: het begeleiden, ondersteunen of adviseren van cliënten, zorggebruikers, ouderen of kwetsbare doelgroepen; het uitvoeren van intakegesprekken, behoefteanalyses, sociaal onderzoek of het opstellen en opvolgen van hulp- of begeleidingsplannen; het coördineren, organiseren of plannen van zorg-, hulpverlenings-, dienstverlenings- of teamactiviteiten; het aansturen, coachen, begeleiden of ondersteunen van medewerkers; het leiden van multidisciplinair overleg en samenwerken met interne en externe partners; het beheren van dossiers en het uitvoeren van administratieve opvolging binnen een zorg-, welzijns- of dienstverlenende context.
  • Leidinggevende ervaring in een gelijkaardige functie is een groot pluspunt.
