Werken bij Pantein Technisch Thuiszorg Verpleegkundige (HBO-V) Thuiszorg Uden 24 - 28 uur Conform CAO Wat ga je doen? Als Technisch Thuiszorg Verpleegkundige (TTV) indiceer, organiseer en verricht je verpleegtechnische zorgtaken bij cliënten met een acute, hoog complexe en/of uitzonderlijke zorgbehoefte. Je hebt nauw contact met huisartsen, specialisten, reguliere wijkverpleging en andere zorgorganisaties met als doel goede verantwoorde zorg met behoud van zelfstandigheid in de thuissituatie. Als Technisch Thuiszorg Verpleegkundige heb je een consult- en adviesfunctie binnen en buiten de eigen organisatie. Opnames in het ziekenhuis worden steeds korter en cliënten kunnen langer en zelfstandiger thuis blijven wonen met de juiste zorg thuis. Hoog complexe zorg en behandelingen vinden daarom steeds vaker thuis plaats. Je bent onderdeel van een organiserend, flexibel en gedreven team, dat bestaat uit meerdere verpleegkundigen. Je hebt een verantwoordelijke functie met veel uitdaging, vrijheid en ontwikkelmogelijkheden. Je bent werkzaam in de volgende gemeenten: Meierijstad, Bernheze, Oss, Maashorst en Boekel. De uitvalbasis bevindt zich in Uden. Je werkzaamheden bestaan uit: - Het verlenen van hoog complexe verpleegtechnische zorg, zoals palliatieve sedatie, infuustherapieën en peritoneale dialyse; - Het verlenen van een achterwachtfunctie voor zorgverleners van Pantein en andere zorgaanbieders uit de regio; - Het geven van voorlichting en instructie om de zelfredzaamheid van de cliënt zoveel mogelijk te stimuleren; - Indiceren, organiseren en coördineren van zorg zodanig dat het behoud van zelfstandigheid en eigen regie van de klant voorop staat; - Bewaken van kwaliteit van de zorg en aanleveren van verbeterpunten; - Participeren in projecten en beleid binnen Pantein om de zorg te optimaliseren; - Het draaien van alle diensten inclusief nachtdiensten met regiopartners.
