The posting

  • Assessment en therapeutische begeleiding van jongeren in functie van vaak voorkomende beperkingen/stoornissen (druggebruik, agressie, sub assertiviteit, zelfbeeld…).
  • Het organiseren en uitvoeren van psycho-educatieve programma’s in groep of individueel.
  • Het gebruiken van psychodiagnostische instrumenten.
  • Het organiseren en uitvoeren van het assessment.
  • Het ondersteunen/coachen van de context in het omgaan en aanpakken van de problematiek van de jongere.

1. Mee opstellen en uitvoeren van het behandelplan.

  • Duidelijk en open communiceren met alle collega’s.
  • Duidelijk en accuraat registreren op de afgesproken wijze en frequentie.
  • Kritisch nadenken over zichzelf en de eigen manier van handelen t.o.v. de collega’s, de cliëntengroep en hun context.
  • Vragen naar en open staan voor feedback van jongeren, context, collega’s en leidinggevenden.
  • Mee instaan voor het aanbrengen van accurate informatie/observaties en het aanbrengen van onderbouwde hypotheses met betrekking tot het therapeutisch en het contextueel luik van het behandelplan.
  • Het professioneel handelen steeds afstemmen op het behandelplan.
  • Indien nodig initiatief nemen om, in overleg met de klinisch coördinator, het behandelplan bij te sturen.

2. Het activeren, verbinden, verbreden en versterken van het netwerk rond de jongere en de context.

  • Het in samenwerking met een collega groepsbegeleider organiseren en uitvoeren van een outreachende nazorg, die gericht is op het verbreden, uitbouwen, versterken van het netwerk rond de jongere en de context.
  • Het netwerk in kaart brengen en op zoek gaan naar mogelijkheden om het netwerk verder uit te bouwen en/of te versterken.
  • De jongere en de context begeleiden en indien nodig doorverwijzen naar de vervolg hulpverlening.
  • Beschikken over voldoende maturiteit.
  • Actief kunnen meewerken in een multidisciplinair kader.
  • Vlot kunnen rapporteren.
  • Bachelor/Master diploma in een menswetenschappelijke richting.
  • Een teamspeler zijn.
  • Open zijn in communicatie en zichzelf in vraag durven stellen.
  • Groot verantwoordelijkheidsgevoel, zelfstandig kunnen werken.
  • Creatieve geest in de begeleiding van jongeren en hun context.
  • Op een constructieve manier kunnen omgaan met crisis en stress.
  • Ervaring in drughulpverlening, groepswerk en of contextueel werk is een voordeel.
  • In het bezit zijn van een rijbewijs cat. B is noodzakelijk.
