The posting

Als verpleegkundige binnen een gestructureerde equipe van verpleegkundigen en zorgkundigen verstrekt u kwalitatief hoogstaande dienstverlening op maat van de patiënt.

U staat in voor het deskundig uitvoeren van verpleegkundige taken in het natuurlijke thuismilieu van de patiënt, steeds rekening houdend met de wensen en behoeften van de patiënt alsook met zijn omgeving. U verzorgt de patiënt in zijn totaliteit (fysisch, psychisch, sociaal, relationeel en geestelijk) en u respecteert de privacy en zelfredzaamheid van de patiënt.

De verpleegkundige respecteert hierbij de rechten van de patiënt en oefent zijn/haar beroep uit volgens de algemeen geldende waarden en normen eigen aan dit beroep en aan de organisatie.

  • Communicatie: met respect voor de privacy en het beroepsgeheim, bouwt u een vertrouwensrelatie op met de patiënt en mantelzorgers. U communiceert respectvol, duidelijk en op maat van de patiënt en zijn mantelzorgers met een open, vriendelijke, empathische houding. U rapporteert beknopt aan de verantwoordelijke en kunt steunen op de verantwoordelijke, collega’s en de referentiepersonen in huis bij vragen of problemen.
  • Zorgverlening: U werkt op een autonome en professionele manier om een gepersonaliseerde totaalzorg aan te bieden aan de patiënt, binnen de geldende procedures en afspraken. U volgt het behandelingsplan op en verwittigt de verpleegkundigen en verantwoordelijke bij nodige wijzigingen. Tijdens de verzorging moeten de algemene hygiënische voorschriften correct gevolgd worden.
  • Administratie: Dagelijks worden observaties en updates doorgegeven via een elektronisch platform.

Zelfontplooiing: U bent bereid om verder te groeien in uw deskundigheid. Er worden geregeld vormingen georganiseerd. Binnen het team is het belangrijk om de eigen bevoegdheden en verantwoordelijkheden goed in te schatten. Er worden regelmatige overlegmomenten voorzien met het team om de werking en de zorg samen te verbeteren en kennis te delen.

  • Verpleegkundige. (of beschikken over een RIZIV nummer of de nodige documenten om dit nummer aan te vragen)
  • Je plaatst de zorgvrager op de eerste plaats.
  • Je bent collegiaal, stipt en ordelijk.
  • Je beschikt over een geldig rijbewijs B.
