The posting

We zoeken een tijdelijke verpleegkundige voor het geriatrisch dagziekenhuis - geriatrisch support team. Je krijgt de kans om vanuit je kennis en passie voor de oudere zorgvrager, mee te werken aan het ziekenhuisbreed verspreiden van een geriatrische zorgcultuur. Je kan samen met een multidisciplinair team bijdragen aan een betere levenskwaliteit voor kwetsbare ouderen.

Op verzoek van de behandelende arts van de zorgvrager, doet het team aan zorgdiagnostiek. Geriatrie is een veelzijdige discipline met vele uitdagingen. De vragen van de oudere zorgvrager gaan voornamelijk over het zelfstandig functioneren, over problemen met het geheugen, de mentale gezondheid, met stappen en het risico op vallen, met medicatie, voeding en slikken,... We vermijden onnodige opnames door onderzoeken goed te plannen en goed samen te werken. Je werkt in daguren tussen 8u - 16u06 en er zijn geen weekenddiensten.

Een blik op jouw takenpakket:

- Je plant en coördineert: van de voorbereiding van opname tot de verpleegtechnische handelingen en de nazorg. - Je ondersteunt collega’s op niet-geriatrische afdelingen en neemt hen mee in de geriatrische zorgcultuur. - Je voert een geriatrische assessment uit op niet-geriatrische afdelingen. - Je werkt procedures uit, op maat van de geriatrische patiënt. - Je werkt samen in multidisciplinair teamverband en je betrekt gemakkelijk andere disciplines voor verdere evaluaties. - Je neemt de rol op van belangenbehartiger van de oudere zorgvrager en je hebt oog voor zijn/haar sociale context. - Je maakt verslaggeving op van de resultaten en aanbevelingen op basis van het geriatrisch assessment voor interne en externe partners.

- In het bezit zijn van een diploma bachelor in de verpleegkunde - In het bezit zijn van een beroepstitel geriatrie. - Een klinische visie en deskundigheid m.b.t. geriatrische problematiek. - Een grote interesse in het verouderingsproces en het levensverhaal van de oudere zorgvrager. - Je bent een teamplayer en bent bereid om intern de rol van educator op te nemen.
