Je analyseert wat er speelt, maakt samen met de cliënt, diens naasten en ketenpartners een plan en zet duidelijke lijnen uit voor de begeleiding. Dat geeft rust, richting en houvast. Binnen Wonen & Zorg begeleiden we cliënten met GGZ-problematiek die tijdelijk of langdurig ondersteuning nodig hebben. Goede begeleiding vraagt om nabijheid, consistentie, samenwerking en professionele regie. Jij houdt overzicht en zorgt dat betrokkenen weten wat de bedoeling is. Wat jouw rol inhoudt Als Trajectbegeleider houd je regie over je casussen. Je stelt zorgplannen op, bewaakt doelen en evalueert periodiek de voortgang. Je analyseert patronen, signaleert risico s en stuurt bij wanneer situaties veranderen. Je bent eerste aanspreekpunt voor behandelaren, ketenpartners en de naasten van de cliënt. Binnen het team zorg je voor richting: je ondersteunt collega s in de uitvoering en bewaakt de samenhang en kwaliteit van de begeleiding. Waar de Trajectondersteuner vooral dicht bij de dagelijkse begeleiding staat, houd jij het grotere geheel in beeld. Je maakt keuzes wanneer dat nodig is en helpt het team om de begeleiding consistent te houden. De context waarin je werkt Je werkt binnen Wonen & Zorg, waar we cliënten ondersteunen die tijdelijk of langdurig niet zelfstandig kunnen wonen. Dit doen we binnen verschillende vormen, zoals beschermd wonen, beschermd thuis, intensief wonen en langdurige zorg. Je biedt begeleiding op maat, afgestemd op wat iemand nodig heeft. Samen werk je met cliënten aan kwaliteit van leven, herstel, zelfstandigheid en het behouden of hervinden van eigen regie.
