Trajectbegeleider B Groningen | 24-32 uur per week | HBO | Schaal 8 (maximaal €5.216,-) | 12 maanden met kans op verlenging Dit ga je doen Team Groningen is een generalistisch team. Wij bieden ambulante begeleiding, Housing First, vangnetopvang en interventiewerk. Voor het interventiewerk zijn wij op zoek naar uitbreiding. Samen met je collega ben je actief op pad in de stad en de regio. Je onderhoudt daarbij contact met zowel cliënten als ketenpartners. Met het interventiewerk geven wij vorm en inhoud aan de outreachende subsidie die LIMOR ontvangt van het programma BW&MO. De opdracht hierbij is; voorkom of doorbreek dakloosheid en wend opvang af. De toeleiding naar (perspectief op) wonen of indien noodzakelijk opvang, inkomen en zorg zijn hierbij essentieel. Nadat er door jouw inzet voorliggende financiering is geregeld bijvoorbeeld in de vorm van een postadres verwijs je voor begeleiding door naar je collega's of een collega organisatie. Ook heb je een belangrijke rol in de toeleiding van Housing First trajecten. Iets specifieker We hebben een flinke wensenlijst. We willen namelijk het beste voor onze cliënten. Wij zijn bevlogen, integer, ondernemend en samenwerkend en verwachten dat nieuwe collega's dit ook zijn en bewijzen in hun werk. Daarnaast noemen we enkele belangrijke punten specifiek voor deze functie. Uiteraard komen er meer ter sprake als we je mogelijk uitnodigen voor een gesprek. Je biedt zelfstandig op methodische wijze ondersteuning en begeleiding aan cliënten en draagt zorg voor de uitvoering en voortgang van vastgestelde doelen. Je bent verantwoordelijk voor het ontwikkelen van het maatwerk-begeleidingsplan, waarvoor je de hulpvraag en behoeften van de cliënt inventariseert. Dit doe je in samenspraak met de cliënt en externe disciplines. Mogelijke knelpunten in de directe hulpverlening en de voortgang signaleer je en bespreek je met je collega's.
