Je analyseert wat er speelt, maakt samen met de cliënt, diens naasten en ketenpartners een plan en zet duidelijke lijnen uit voor de begeleiding. Dat geeft rust, richting en houvast. Binnen Wonen & Zorg begeleiden we cliënten met GGZ-problematiek die tijdelijk of langdurig ondersteuning nodig hebben. Goede begeleiding vraagt om nabijheid, consistentie, samenwerking en professionele regie. Jij houdt overzicht en zorgt dat betrokkenen weten wat de bedoeling is. Locatie Händellaan Op de locatie Händellaan bieden wij wonen en zorg onder één dak voor cliënten die langdurig permanente nabijheid nodig hebben. Deze zorg wordt geleverd op basis van de Wet Langdurige Zorg (WLZ). Het doel van Langdurige Zorg is cliënten te leren om te gaan met hun kwetsbaarheid en het begeleiden bij en het hanteren van de effecten van gedrag. De begeleiding richt zich op het behouden en waar mogelijk het vergroten van stabiliteit. De focus ligt op behoud van lichamelijke-, psychische- en psychosociale gezondheid en er is aandacht voor stabiliteit en ontwikkeling. Er kan echter ook sprake zijn van begeleiding bij achteruitgang en het voorkomen van instabiliteit op het gebied van levensterreinen zoals wonen, lichamelijke verzorging, financiën en sociale relaties. Er wordt toegewerkt naar een zinvolle dag invulling voor elke cliënt. Er is 24 uur per dag toezicht op de locatie aanwezig. Wat jouw rol inhoudt Als Trajectbegeleider houd je regie over je casussen.
