Word jij onze triagemedewerker Toegang Jeugd bij RondomJou? Wat is RondomJou? RondomJou is een samenwerkingsverband van Xtra (JESS en MEE ZHN), Jeugdformaat, Youz en Middin. Samen bundelen wij onze kennis en expertise op het gebied van jeugd- en opvoedhulp, GGZ, welzijnswerk en ondersteuning aan mensen met een licht verstandelijke beperking. Hoe werken wij? Vanuit onze buurtteams, het onderwijsteam en de Toegang ondersteunen wij jeugdigen en gezinnen in de Haagse wijken Haagse Hout, Laak, Leidschenveen-Ypenburg en Centrum. Samen met ouders, scholen, huisartsen en andere partners zoeken we naar oplossingen die aansluiten bij de leefwereld van gezinnen. Waar nodig zetten we aanvullende ondersteuning in. Altijd binnen het vrijwillig kader en met oog voor wat écht nodig is Dit ga je doen Als Triagemedewerker Toegang Jeugd ben jij de eerste schakel in het hulpverleningsproces. Je beoordeelt aanvragen, onderzoekt hulpvragen en zorgt ervoor dat jeugdigen en gezinnen snel de meest passende ondersteuning krijgen. Jouw werkzaamheden bestaan onder andere uit: - Het voeren van telefonische intakes en screenings. - Het analyseren van hulpvragen en het verzamelen van relevante informatie. - Het bepalen van de meest passende hulproute voor jeugdigen en gezinnen. - Het adviseren van ouders, verwijzers en collega's over de mogelijkheden binnen en buiten rondomJou. - Het beslissen over benodigde interventies en specialistische inzet. - Het beoordelen van spoedmeldingen en ...
