Mozaïek biedt woon- en dagondersteuning en individuele begeleiding aan mensen met een beperking. Wij verbinden mensen en maken hen krachtiger. Daartoe ontwikkelden we een heldere visie op cliëntgerichte hulpverlening, hartelijke kwaliteitsvolle zorg en gedeeld leiderschap.

Om onze missie te kunnen verwezenlijken, zoeken we een meewerkend teamcoach voor ons paramedisch team die verpleegkundige deskundigheid combineert met coachend leiderschap en die samen met het team onze medische en paramedische werking verder vormgeef

JOUW ROL: DESKUNDIG, VERBINDEND EN COÖRDINEREND

Binnen het paramedisch team werk je nauw samen met andere diensten in Mozaiek aan een holistische en multidisciplinaire begeleiding van onze cliënten.

Als meewerkend teamcoach combineer je coachend leiderschap met actieve betrokkenheid in de dagelijkse verpleegkundige werking. Je organiseert en versterkt het paramedisch team, bouwt mee aan de verdere ontwikkeling van de medische werking en ondersteunt als teamcoach ook de verschillende werkingen van Mozaïek (dagondersteuning, wonen en mobiele begeleiding).

Wat je doet:

· Je coacht medewerkers individueel, benut hun sterktes en ondersteunt hen in hun professionele groei.

· Je staat in voor de planning en organisatie van het paramedisch team en bewaakt mee de continuïteit van de werking.

· Je analyseert noden en knelpunten binnen de medische en paramedische werking en vertaalt deze naar concrete verbeteracties.

· Je werkt mee aan het ontwikkelen, vormgeven, implementeren en verbeteren van het medische en paramedische beleid binnen Mozaïek.

· Je verdiept je in relevante wetgeving en kwaliteitsvereisten en vertaalt deze naar duidelijke procedures en werkbare afspraken.

· Je werkt protocollen en procedures verder uit en volgt deze op, onder andere rond epilepsie, medicatiebeleid en signaleringsplannen.

· Je stimuleert kennisdeling en zorgt ervoor dat relevante vormingen voor medewerkers worden ontwikkeld en gegeven door het paramedisch team.

· Je bewaakt mee een kwaliteitsvol medicatiebeleid en een goede medische opvolging van onze cliënten.

· Als meewerkend teamcoach blijf je dicht bij de praktijk en neem je ook verpleegkundige taken op.

Een functie met veel variatie, verantwoordelijkheid en menselijk contact, waarin je schakelt tussen coachen, organiseren, beleid vormgeven en zelf meewerken in de verpleegkundige praktijk.

· een diploma Verpleegkunde (HBO5 of Bachelor);

· ervaring in of duidelijke affiniteit met het coachen en ondersteunen van medewerkers;

· sterke communicatieve vaardigheden en empathie;

· het vermogen om mensen te verbinden, maar ook duidelijkheid en richting te geven waar nodig;

· een planmatige en nauwkeurige werkstijl en sterke organisatorische vaardigheden;

· interesse in wetgeving, kwaliteitsbeleid en de verdere professionalisering van een medische en paramedische werking;

· een kritische en constructieve blik: je ziet waar zaken beter kunnen en neemt initiatief om samen tot oplossingen te komen;

· voeling met de leefwereld en ondersteuningsnoden van mensen met een beperking;
