Wit-Gele Kruis West-Vlaanderen staat voor kwaliteitsvolle thuisverpleging. We spelen optimaal in op de totale zorgnood, zodat mensen met de grootst mogelijke autonomie zo lang mogelijk in de eigen vertrouwde omgeving kunnen blijven. Onze Zorgcentrale ondersteunt mee deze doelstelling. De oproepen die we vanuit dit personenalarmsysteem ontvangen, worden vanuit onze verpleegkundige expertise opgevolgd.

Ter versterking van dit team gaan wij op zoek naar een:

Verpleegkundig medewerker Zorgcentrale (deeltijds, 28.5u of 32u)

Een greep uit jouw verantwoordelijkheden:

  • Je bent als zorgverlener verantwoordelijk voor de professionele afhandeling en opvolging van binnenkomende meldingen over personenalarmering.
  • Op basis van afhandelingsprotocollen maak je een deskundige beoordeling van de meldingen en onderneem je de gepaste actie: je biedt telefonisch een oplossing of linkt de zorgvraag met de meest aangewezen zorgverlener.
  • Je staat ook in voor nazorg van de meldingen en opvolgcommunicatie met de patiënt, eventueel mantelzorger, huisarts of thuisverpleegkundige. Je denkt daarbij oplossingsgericht en doet ook proactief voorstellen om bijvoorbeeld herhaling van meldingen in de toekomst te vermijden door de nodige zorg te organiseren.
  • Ook ondersteun je administratief mee binnen de werking: inbrengen aanvraagformulier, opvolging werking van de toestellen, enz…
  • Je beschikt over een diploma verpleegkunde.
  • Ervaring in thuisverpleging, in een alarmcentrale of op kritische diensten (spoed, intensieve zorgen…) is een mooie meerwaarde maar zeker geen must.
  • Je bent communicatief en kan op een professionele manier in gesprek gaan met de klant/patiënt, met de mantelzorger, behandelende arts en/of andere partners in zorg.
  • Je verpleegkundige expertise stelt je in staat om vanop afstand zelfstandig de situatie van de patiënt en zijn zorgvraag correct in te schatten en passende acties (zowel reactief als proactief) te nemen of te coördineren en verder op te volgen.
  • Je bent erg nauwkeurig en houdt van administratief werk.
  • Je goede stressbestendigheid maakt dat je crisissituaties rustig en professioneel weet aan te pakken.
  • Je werkt vlot en graag met de computer en courante Office-tools.
  • Je kunt je ook in Frans en Engels vrij vlot uit de slag trekken.
  • Wij kijken uit naar een deeltijdse tewerkstelling maar het exacte regime is bespreekbaar (28.5u à 32u maximum per week). Dit gaat om een tewerkstelling in flexibele diensten.
  • Dit kan variëren tussen vroege diensten (06u00 tot 14u30), dagdiensten (07u00 tot 15u30) en late diensten (14u00 tot 22u00). Ook kunnen hier op 1 op de 2 weekenddiensten inzitten. Deze planning is echter ruim vooraf gekend en wordt opgemaakt in samenspraak.
