- Verpleegkundig Specialist óf POH-Ouderen | 1 dag per week Heb jij hart voor ouderenzorg én voor persoonlijke huisartsenzorg? Dan maken we graag kennis met je! Voor Huisartsenpraktijk G.E. Bolsius in Hoofddorp zoeken wij een Verpleegkundig Specialist of POH-Ouderen die samen met ons de zorg voor onze oudere patiënten verder wil vormgeven. Wij zijn een kleinschalige en betrokken huisartsenpraktijk waar we elkaar kennen, korte lijnen belangrijk vinden en persoonlijke aandacht voor onze patiënten centraal staat. Wat ga je doen? Je krijgt binnen onze praktijk een zelfstandige en belangrijke rol in de ouderenzorg. Je: - begeleidt en monitort kwetsbare en oudere patiënten; - houdt spreekuur en legt waar nodig huisbezoeken af; - kijkt niet alleen naar de medische situatie, maar naar de patiënt als geheel; - onderhoudt contact met familie en mantelzorgers; - werkt samen met thuiszorg, apotheek, ziekenhuis en andere zorgverleners in de wijk; - coördineert de zorg rondom kwetsbare ouderen; - krijgt ruimte om mee te denken over én vorm te geven aan de ouderenzorg binnen onze praktijk. Wie ben jij? Je bent een Verpleegkundig Specialist of POH-Ouderen met affiniteit voor ouderenzorg. Je werkt zelfstandig, bent proactief en houdt ervan om zaken goed te organiseren. Daarnaast vinden we het belangrijk dat je: - betrokken en patiëntgericht bent; - makkelijk communiceert met patiënten, naasten en collega's; - initiatief neemt; - goed kunt samenwerken; - plezier hebt in je werk en bij een persoonlijke, kleinschalige praktijk past. Ervaring in de eerste lijn is fijn, maar niet het belangrijkste. We vinden vooral belangrijk wie jij bent en hoe jij naar patiënten en zorg kijkt.
