Wij hebben een vacature voor een professionele en enthousiaste verpleegkundig specialist die, met zelfstandigheid, cliënten ondersteunt in hun herstelprocessen. Herken jij jezelf hierin? Dan is deze vacature wellicht iets voor jou! Elke ochtend begint het wijkteam met een overleg bij het digibord. Er worden dan cliënten besproken die direct de aandacht nodig hebben. Na deze bespreking gaat iedereen uit het team aan de slag met zijn eigen caseload. Jij als verpleegkundig specialist GZ deelt je eigen werkweek in en beslist samen met de cliënt wanneer je iemand ziet. Dit kan op onze locatie in Voorhout zijn, maar ook bij de cliënt thuis. Naast je een eigen caseload, ben je ook regiebehandelaar van een aantal cliënten. Je bent zelfstandig in staat om een diagnose te stellen, waarna je ook het behandelplan opstelt. Hierbij houd je rekening met zowel psychische, medische en somatische klachten. Je monitort continu of de voorgenomen handelingen het gewenste effect hebben op de cliënt. Indien nodig pas je deze ad-hoc aan. En verder? Ben je medeverantwoordelijk voor de inhoudelijke ontwikkeling op het verpleegkundig vakgebied en werkt nauw samen met het verpleegkundig team.
