Jij zorgt ervoor. Zo werk jij Als verpleegkundig specialist binnen de Kliniek Ouderenpsychiatrie ben jij er voor ouderen met psychiatrische problematiek, vaak in combinatie met cognitieve achteruitgang of somatische aandoeningen. Jij combineert medische deskundigheid met persoonlijke aandacht en zorgt voor een veilig en vertrouwd behandelklimaat. Vanuit jouw expertise en persoonlijke talenten geef jij vorm aan de functie van verpleegkundig specialist. Als regiebehandelaar ben je verantwoordelijk voor het opstellen, coördineren en uitvoeren van behandelplannen, waarbij je samen met je collega's zorg op maat biedt. De belangrijkste taken en verantwoordelijkheden zijn voor jou bekend terrein. Wat de functie bij GGz Centraal bijzonder maakt is: - het nemen van de regie in de afstemming en nauwe samenwerking met familie en naasten; - het organiseren en voorzitten van het Zorgafstemmingsoverleg (ZAG), waarbij jij de verbindende factor bent; - het voeren van intakegesprekken, diagnostiek en het voorschrijven van medicatie; - het dragen van de regiebehandelaar rol en het bewaken van de voortgang van behandelingen; - het begeleiden en ondersteunen van zowel cliënten als hun naasten; - samenwerken in een multidisciplinair team en bijdragen aan innovatie van de zorg; - supervisie en coaching van collega's en verpleegkundigen in opleiding.
