28 tot 36 uur Heb jij als zorgprofessional hart voor de psychiatrie? Als verpleegkundig specialist maak jij het verschil voor cliënten op de polikliniek NCS voor ouderen en neuropsychiatrie. Met de juiste diagnose en een optimale samenwerking lever jij kwalitatief hoogwaardige en cliëntgerichte zorg. Aandacht voor persoonlijke groei Jij bent als regiebehandelaar verantwoordelijk voor de behandeling van cliënten met klachten op het gebied van neuropsychiatrie. Je coördineert, samen met onder andere de psychiater en klinisch psycholoog, de zorg rondom de cliënt en het netwerk, maar bent ook kartrekker als het gaat om beleidsontwikkelingen. Dat doe je samen met het team, dat met zorg voor elkaar de beste zorg voor onze cliënten levert. Wat je verder gaat doen: - Je doet intakes en voert behandeling uit met daarbij diagnostiek. Hierbij stel je het zorgplan op in nauwe samenspraak met cliënt, netwerk en het multidisciplinaire team. - Je voert psychiatrisch en somatisch diagnostisch onderzoek uit volgens de geldende richtlijnen. Uiteraard heb je veel contact met verwijzers en interne en externe hulpverleners. - Je coördineert de zorg rondom de cliënt en het netwerk en stuurt andere behandelaren aan die betrokken zijn in de zorg. - Je schrijft medicatie voor en overlegt wanneer nodig met de psychiater. - Je voert, wanneer nodig, lichamelijk onderzoek uit en interpreteert labgegevens. - Je werkt individueel, maar weet je teamgenoten en collega's in andere teams te vinden. Jouw werkplek Je gaat impact maken in de polikliniek NCS voor ouderen en neuropsychiatrie in het hart van Gorinchem. Er zijn op deze locatie meerdere poliklinieken gehuisvest, waardoor de lijnen met verschillende collega's kort zijn. Samen maken we het verschil door multidisciplinair samen te werken.
