Solliciteren voor "Verpleegkundig Triagist" Verpleegkundig Triagist Regio Noord-Holland FWG 50 24 uur Verpleegkundig Triagist Wil jij als verpleegkundig triagist bijdragen aan het slim en efficiënt verbinden van (sub)acute zorgvragen met regionale zorg? Jouw nieuwe baan Als verpleegkundig triagist speel je een belangrijke rol binnen het Coördinatiepunt Ouderenzorg Noord-Holland. Samen met alle zorgpartijen in de regio zorgen we ervoor dat kwetsbare ouderen snel de juiste zorg op de juiste plek krijgen. Hierdoor benutten we de regionale capaciteit optimaal en voorkomen we onnodige wachttijden, zowel in acute situaties als na ziekenhuisopnames. Je werkt in een regionaal team van triagisten vanuit meerdere VVT-organisaties, verspreid over verschillende locaties in de Kop van Noord-Holland, West-Friesland, Noord-Kennemerland, Waterland en de Zaanstreek. De diensten draaien we volgens een wisselend rooster op werkdagen tussen 08.00 en 20.00 uur. Voor spoedsituaties is het Coördinatiepunt 24/7 bereikbaar; in de avond-, nacht- en weekenduren neemt de zorgservicecentrale de waarneming over. In jouw rol ondersteun je verwijzers, stel je passende indicaties en zorg je voor de toeleiding naar de juiste zorg. Je helpt verwijzers bij het inschatten van de (sub)acute zorgvraag en faciliteert hen bij het organiseren van eerstelijnsverblijf, crisisopnames (WLZ) en acute thuiszorg. Je bemiddelt naar alle VVT-aanbieders in de regio, altijd met de cliënt en diens netwerk centraal. Je maakt 'de vraag achter de vraag' zichtbaar, verzamelt de benodigde informatie via het afwegingsinstrument Eerstelijnsverblijf en vertaalt dit naar deskundig advies en passende zorgbemiddeling.
