Evean ziet mogelijkheden voor iedereen. Met (wijk)verpleging, dagbesteding, verzorgd wonen, revalidatie, (zorg)ondersteuning en welzijn draagt Evean bij aan een zo goed mogelijk leven voor iedere cliënt en bewoner. Het Coördinatiepunt Ouderenzorg NH

Het Coördinatiepunt Ouderenzorg is een samenwerking voor en met alle zorgpartijen in de regio en werkt onafhankelijk en neutraal. Binnen het Coördinatiepunt Ouderenzorg Noord-Holland maken alle zorgpartijen zich sterk om voor kwetsbare ouderen in het hele verzorgingsgebied de juiste zorg op de juiste plek te bieden. Hierdoor wordt de gezamenlijke capaciteit beter benut en wordt voorkomen dat kwetsbare ouderen onnodig lang moeten wachten op de juiste zorg in een acute situatie of na een ziekenhuisopname.

Jij komt in dienst bij Evean en werkt samen met triagisten van andere zorgorganisatie in de regio. Er wordt vanuit verschillende locaties en verspreid over de regio’s de kop van Noord-Holland, West-Friesland, Noord-Kennemerland, Waterland en Zaanstreek gewerkt.

De diensten zijn volgens wisselend rooster op werkdagen tussen 8:00 en 20:00 uur. Het coördinatiepunt is 24/7 bereikbaar voor spoed, tijdens de avond- nacht en weekenduren vindt de waarneming plaats door een andere partij.
