Als verpleegkundig zorgcentralist in Varsseveld combineer je de coördinatie van eerstelijns verblijf (ELV) met het monitoren van patiënten op afstand. Zo zorg jij ervoor dat mensen de juiste zorg krijgen, op het juiste moment en op de juiste plek. Voor de ELV-coördinatie ben jij de spil in het organiseren van een passende vervolgplek voor patiënten die tijdelijk niet thuis kunnen wonen. Je brengt de zorgvraag in kaart, schakelt met zorgaanbieders, ziekenhuizen en andere ketenpartners en begeleidt het plaatsingsproces van begin tot eind. Daarnaast monitor je op afstand de gezondheid van patiënten van ziekenhuizen en huisartsen. Je beoordeelt meetwaarden en gezondheidsvragenlijsten, signaleert bijzonderheden en neemt contact op wanneer dat nodig is. Je ondersteunt patiënten in het vergroten van hun eigen regie en zelfmanagement door voorlichting, coaching en inzicht in hun eigen ziektebeeld. Samen met huisartsen, verpleegkundig specialisten, medisch specialisten en andere zorgprofessionals zorg je voor passende vervolgzorg. Dankzij jouw deskundigheid worden gezondheidsproblemen vroegtijdig gesignaleerd en kunnen tijdig de juiste interventies worden ingezet, waardoor passende zorg op het juiste moment wordt geboden.
