Wil jij als psychiatrisch verpleegkundige werken in de acute psychiatrie én meebouwen aan de uitbreiding van IHT? Als verpleegkundige binnen het IHT biedt je intensieve, kortdurende crisisbehandeling (maximaal 6 weken), zoveel mogelijk in de thuissituatie. Je helpt crisissen stabiliseren, voorkomt waar mogelijk opnames en maakt direct verschil op de momenten die ertoe doen. Je werkt nauw samen met cliënten, hun netwerk en ketenpartners. Samen leveren jullie snelle, doelgerichte en mensgerichte zorg. Tegelijkertijd krijg je volop ruimte om mee te denken over de verdere ontwikkeling van IHT en de samenwerking tussen de drie locaties. Wat ga je doen? - Je verleent en coördineert verpleegkundige zorg en begeleiding aan cliënten met uiteenlopende en complexe psychiatrische problematiek; - Je observeert en bewaakt zowel de psychische als lichamelijke toestand van cliënten en signaleert veranderingen tijdig; - Je draagt bij aan het opstellen, uitvoeren en evalueren van behandel- en herstelplannen; - Je bezoekt cliënten in hun thuissituatie en levert daar intensieve, passende zorg; - Je werkt nauw samen met collega's binnen het multidisciplinaire team en met naasten en ketenpartners; - Je zorgt voor zorgvuldige verslaglegging en werkt volgens geldende richtlijnen en protocollen; - Je draagt actief bij aan de opbouw en ontwikkeling van het nieuwe IHT-team; - Ben je ook inzetbaar voor ons Spoedpunt. Binnen onze Intensive Home Treatment (IHT)-zorg zijn we volop in beweging. IHT is al gevestigd in Leiden en Gouda, en we breiden nu uit met een nieuwe uitvalsbasis in Leidschendam.
