Werken bij Verpleegkundige HBO, Sneek Je kunt reageren tot 9 november 2026 - Verpleegkundige HBO, Sneek - Werkplek: Fact+ - Opleiding: HBO - Dienstverband: 32 - 36 uur per week - Salaris: FWG 50 Je kunt reageren tot 9 november 2026 Regelmatig gaan psychische klachten en verslavingen hand in hand en versterken ze elkaar. In Sudwest Fryslan en de Fryske Marren hebben GGZ Friesland en Verslavingszorg Noord Nederland (VNN) daarom de krachten gebundeld in een aantal zogenoemde FACT+ teams. In deze teams werken experts op het gebied van psychiatrie én verslavingszorg samen om cliënten met een psychiatrische stoornis en/of verslavingsproblematiek een geïntegreerde behandeling te bieden. Hiermee werken we, samen met de cliënt, vanuit één team, één verhaal, één dossier en één plan aan maximale herstelkracht. Voor het team FACT+ team Sneek stad zijn wij op zoek naar een enthousiaste en bevlogen Verpleegkundige HBO. Wat ga je doen en wat staat je te wachten? Als verpleegkundige bij FACT+ heb je een belangrijke rol in het herstelproces van cliënten. Je inventariseert klachten, vragen en herstelwensen ten behoeve van de intake, indicatiestelling, diagnostiek en behandeling. Daarbij werk je volgens de FACT-methodiek en de meest actuele inzichten op het gebied van herstelondersteunende zorg. Naast de behandeling van psychiatrische en verslavingsproblematiek heb je structureel aandacht voor de somatische gezondheid en leefstijl van cliënten. Samen met de cliënt werk je aan herstel, waarbij je nauw samenwerkt met familie en andere naastbetrokkenen. Ook onderhoud je contacten met collega's, verwijzers en andere betrokken instanties om de zorg optimaal op elkaar af te stemmen. Je werkt voornamelijk ambulant en gaat regelmatig bij cliënten thuis langs. Je houdt je bezig met onder andere casefinding, bemoeizorg en zorgtoeleiding en werkt daarbij vanuit de OGGZ intensief samen met lokale ketenpartners. Waar nodig kun je terugvallen...
