Over je functie Als verpleegkundige zorg je dat de zorg op de groepswoningen goed aansluit bij de bewoners. Je houdt overzicht, stemt af met collega's en behandelaren en betrekt familie en naasten. Tegelijk werk je mee in de dagelijkse zorg. Zo zie je zelf wat bewoners nodig hebben. Dit ga je doen • Je coördineert de zorg rondom bewoners. • Je stelt zorgplannen op en houdt deze actueel, samen met de bewoner en diens naasten. • Je signaleert veranderingen in gezondheid of gedrag en onderneemt actie. • Je stemt de zorg af met collega's, behandelaren en familie. • Je ondersteunt collega's bij vragen over de zorg. • Je biedt zelf zorg en begeleiding aan bewoners. • Je draagt bij aan een veilige en huiselijke plek om te wonen.
