Nieuwe werkwijze bij TSN Zorg een betere werk-privé balans Werken bij TSN Mensen met dementie kunnen steeds langer thuis blijven wonen. Hiervoor is professionele en kwalitatief goede zorg nodig, zowel voor de cliënt als voor zijn of haar netwerk er om heen. Als Casemanager Dementie bied je ondersteuning aan mensen met (beginnende) dementie, maar ook aan de mantelzorgers. Je bent hierbij in staat te informeren, te ondersteunen en te adviseren, en sluit hierbij aan op de verschillende vormen van dementie. Voor de regio Hoogezand-Sappemeer zijn wij, vanwege ziektewaarneming, per direct op zoek naar een Casemanager Dementie voor 24-32 uur/week Over deze leuke baan Je bent een ervaren Verpleegkundige (Hbo) met passie voor het vak. Daarnaast heb je Post-HBO opleiding casemanagement dementie succesvol afgerond. Naast de opleidingseisen verwachten wij dat o.a. het volgende meebrengt: - Je bent in staat methodisch te werken en te zorgen voor een goed verloop van het zorgverleningsproces. Hierbij ben je in staat samen te werken met verschillende disciplines en is een nauwe samenwerking met de thuiszorgteams van belang. - Je beschikt over goede communicatieve vaardigheden -zowel mondeling als schriftelijk-, inlevingsvermogen en je bent in staat motiverend te overtuigen en te bemiddelen waar nodig. - Je maakt gebruik van de kennis over dementie die je uit ervaring en opleiding hebt opgedaan. Je (her)kent hierdoor de verschillende fases van dementie, de voortgang én de prognose van verschillende complexe casuïstiek. Voor in de regio zijn wij met name op zoek naar iemand met ervaring met cliënten met diverse achtergronden die de Nederlandse taal niet altijd vaardig zijn. Wat ga jij doen? - Intensieve begeleiding geven aan de persoon met dementie en diens mantelzorgers - Het geven van psycho-educatie aan de persoon met dementie en diens steunsysteem - Daar waar mogelijk de zorg zo inrichten dat optimaal gebruik wordt gemaakt van de eigen mogel...
