Voor ons thuiszorgteam in Hoorn zijn wij op zoek naar een betrokken Verpleegkundige. Samen zorgen we ervoor dat cliënten zo lang mogelijk prettig, veilig en zelfstandig thuis kunnen blijven wonen. Al werkzaam bij Calidus? Solliciteren kan gewoon via het formulier! Wat ga je doen Werken in de wijk als Verpleegkundige betekent veel vrijheid en verantwoordelijkheid. Je gaat zelfstandig op route, neemt beslissingen en krijgt het vertrouwen om vanuit jouw vakmanschap te handelen. Tegelijkertijd sta je er nooit alleen voor. Je maakt deel uit van een hecht en deskundig team waarin de lijnen kort zijn en collega's altijd bereid zijn om met je mee te denken. Bij iedere cliënt beoordeel je de situatie, bied je passende verpleegkundige zorg en kijk je wat iemand nodig heeft om zo zelfstandig mogelijk thuis te kunnen blijven wonen. Je hebt oog voor veranderingen in de gezondheid, het welzijn en de thuissituatie en onderneemt actie wanneer dat nodig is. Daarnaast: - Stel je samen met de cliënt en het zorgteam het zorgleefplan op en bewaak je de uitvoering hiervan; - Verricht je verpleegtechnische handelingen en houd je de gezondheid van cliënten goed in de gaten; - Signaleer je veranderingen, rapporteer je zorgvuldig en evalueer je de geboden zorg; - Coach en ondersteun je collega's bij verpleegkundige vraagstukken; - Draag je actief bij aan een fijne samenwerking binnen het team en met mantelzorgers en andere zorgprofessionals. Geen dag is hetzelfde. Iedere cliënt heeft een eigen verhaal, een eigen zorgvraag en eigen wensen. Juist die afwisseling, het persoonlijke contact en de mogelijkheid om echt iets te betekenen ...
