Je bent regisseur van het zorgproces, coach voor je collega's en een belangrijke gesprekspartner voor bewoners, familie en behandelaren. Met jouw klinische blik, coördinerende kwaliteiten en hart voor de ouderenzorg zorg je ervoor dat bewoners iedere dag de best mogelijke zorg ontvangen. Zo maak jij het verschil onze voor bewoners en voor het team. Wat ga je doen? Als Hbo-verpleegkundige coördineer je de zorg rondom bewoners met complexe zorgvragen. Je bewaakt de kwaliteit en continuïteit van de zorg, denkt mee over verbeteringen en ondersteunt collega's in hun dagelijkse werk. Daarbij combineer je coördinerende taken met het verlenen van verpleegkundige zorg. Je houdt je onder andere bezig met: - Coördineren, bewaken en evalueren van zorg- en revalidatieplannen. - Toepassen van klinisch redeneren in complexe zorgsituaties. - Signaleren van veranderingen in de zorgvraag en hierop anticiperen. - Uitvoeren van verpleegkundige en verpleegtechnische handelingen. - Coachen, begeleiden en ondersteunen van collega's, leerlingen en stagiaires. - Initiëren van kwaliteitsverbeteringen en bijdragen aan deskundigheidsbevordering binnen het team. - Samenwerken met artsen, behandelaren, kwaliteitsverpleegkundigen en andere disciplines. - Voeren van familiegesprekken en vertegenwoordigen van het verpleegkundig perspectief in multidisciplinaire overleggen. - Meedenken over zorginnovatie, digitalisering en toekomstbestendige ouderenzorg.
