Voor een betrokken ouderenzorgorganisatie in de regio Gorinchem zoeken wij een Verpleegkundige die ook EVV-taken uitvoert rondom bewoners voor 8 tot 24 uur per week. In deze functie combineer je verpleegkundige zorg met het coördineren van zorgprocessen rondom bewoners. Je krijgt verantwoordelijkheid, werkt nauw samen met collega's en draagt dagelijks bij aan een prettig leven voor ouderen. Jouw taken: - Het verlenen van verpleegkundige zorg en het uitvoeren van voorbehouden handelingen; - Het bewaken van de continuïteit en kwaliteit van zorg; - Opstellen, uitvoeren en evalueren van zorg leef plannen; - Onderhouden van contact met cliënten, familieleden en collega's; - Signaleren van veranderingen en bespreken van bijzonderheden met de zorgcoördinator; - Begeleiden van leerlingen en stagiairs. Over de organisatie Je komt terecht bij een warme ouderenzorgorganisatie waar cliënten centraal staan en de lijnen kort zijn. Binnen kleinschalige woonvormen wordt gewerkt vanuit vertrouwen, samenwerking en betrokkenheid. Medewerkers krijgen ruimte om verantwoordelijkheid te nemen, zich te ontwikkelen en mee te denken over de kwaliteit van zorg. De organisatie kent een lokaal en dorps karakter. - Contract voor 8-24 uur per week; - Salaris conform CAO VVT, FWG 45; - Een contract voor de duur van 1 jaar, met uitzicht op een vast dienstverband; - Goede secundaire arbeidsvoorwaarden; - Opleidings- en ontwikkelmogelijkheden.
