Wil jij patiënten begeleiden tijdens een moeilijk periode in hun leven en een belangrijke schakel zijn binnen een hecht team? Dan is deze functie op ons Hoofdpijncentrum iets voor jou. Binnen de polikliniek zie je patiënten met verschillende hoofdpijnklachten. Jij bent voor hen een vertrouwd aanspreekpunt en zorgt ervoor dat zij zich gehoord, ondersteund en goed geïnformeerd voelen gedurende hun zorgtraject. Samen met de neurologen, verpleegkundig specialist en verpleegkundige draag je bij aan passende, persoonlijke en hoogwaardige hoofdpijnzorg. Jouw nieuwe baan Als gespecialiseerd verpleegkundige van het Hoofdpijncentrum ben je verantwoordelijk voor de coördinatie, uitvoering en begeleiding van specialistische zorg. Je werkt nauw samen met neurologen, verpleegkundig specialisten en andere verpleegkundigen. Je houdt je onder andere bezig met: - Het begeleiden, informeren en adviseren van patiënten en hun naasten. - Het uitvoeren van verpleegkundige intakes en het inventariseren van de zorgvraag. - Het coördineren van zorgtrajecten en afstemmen van zorg met interne en externe zorgverleners. - Het signaleren van veranderingen in de gezondheidssituatie en hier adequaat op anticiperen. - Het bieden van psychosociale ondersteuning aan patiënten en hun naasten. - Het leveren van een actieve bijdrage aan kwaliteitsverbetering, protocollen en zorginnovatie. - Het begeleiden van studenten, stagiaires en collega's in opleiding.
