Op Passage, de observatie- en diagnostiekafdeling van GP+, werk je met bewoners bij wie nog niet altijd duidelijk is welke zorg, begeleiding of woonvorm het beste past. Juist daarom zijn jouw verpleegkundige blik, klinisch redeneren en observatievermogen van grote waarde. Wil jij werken in een dynamische omgeving waar je meedenkt over complexe zorgvragen én bijdraagt aan de juiste plek voor iedere bewoner? Dan maken we graag kennis met je. Wat ga je doen Als verpleegkundige op Passage begeleid en verzorg je bewoners met het syndroom van Korsakov en bewoners met een gerontopsychiatrische+ zorgvraag. Vaak verblijven bewoners tijdelijk op de afdeling om te onderzoeken welke ondersteuning en woonomgeving het beste aansluiten bij hun behoeften. Je voert verpleegkundige handelingen uit, coördineert de zorg en observeert veranderingen in gedrag, gezondheid en welzijn. Op basis van jouw bevindingen lever je een belangrijke bijdrage aan de beeldvorming rondom bewoners en denk je mee over passende vervolgzorg. Je werkt nauw samen met verzorgenden, woonbegeleiders, artsen, psychologen en andere behandelaren. Dankzij de korte lijnen binnen het multidisciplinaire team wordt jouw expertise actief meegenomen in het opstellen en evalueren van zorg- en behandelplannen. Daarnaast draag je bij aan de beweging van zorg naar gewoon leven. Samen met bewoners, naasten en collega's kijk je niet alleen naar wat iemand nodig heeft aan zorg, maar vooral naar wat belangrijk is voor kwaliteit van leven, eigen regie en zelfredzaamheid.
