ZAS, De drijvende kracht voor gezondheidszorg in beweging. Drijf je mee?

ZAS is op zoek naar een medewerker voor het mobiel psychiatrisch team voor langdurige zorg.

Het team is actief in de kernstad Antwerpen en biedt behandeling en begeleiding in de thuissituatie en probeert hierdoor opname vermijdend of verkortend te werken. Het zorgaanbod is gericht op volwassenen met een ernstige en complexe psychiatrische problematiek.

WAT?

  • 38 uur/week (100%)
  • Je werkt op weekdagen van 9u-17u. Op indicatie kan hier sporadisch van afgeweken worden vanuit een zorg gerelateerde vraag.

OVER DE JOB

  • Je biedt begeleiding en behandeling in de thuisomgeving van volwassen cliënten (18+) met een psychiatrische kwetsbaarheid. Deze cliënten zijn woonachtig in Antwerpen. De doelgroep wordt gekenmerkt door een mix van problemen zowel op vlak van somatische, geestelijke gezondheidszorg als op vlak van sociaal en maatschappelijk functioneren.
  • Je bouwt een vertrouwensrelatie op met de cliënt en zijn professioneel en niet-professioneel netwerk. Wanneer er nog geen netwerk is, zet je in op de uitbouw ervan (oa. door nauwe samenwerking met eerstelijnsactoren: mantelzorgers, huisartsen, CAW, ambulante centra,… ) om ‘zorg op maat’ en ‘continuïteit van zorg’ te realiseren.
  • Je bevordert mee de deskundigheid inzake psychosociale en geestelijke gezondheidszorg bij de verschillende betrokken partijen rond de cliënt. Dit door de zorgafstemming tussen de verschillende partijen te bevorderen, eraan te participeren en coaching te bieden waar nodig/gewenst.

OVER JOU

  • Je bent in het bezit van een diploma psychiatrisch verpleegkundige.
  • Je hebt minstens 2 jaar ervaring in de geestelijke gezondheidszorg.
  • Je bent communicatief vaardig en beschikt over het nodige zelfinzicht. Je bent in staat om duidelijk te rapporteren aan de leden van het multidisciplinair team.
  • Je bent sterk in het plannen en organiseren. Je bent pragmatisch ingesteld en hebt zin voor initiatief.
  • Je bent stressbestendig en flexibel, ook wat betreft je werkuren.
  • Je kan omgaan met diversiteit.
