Je biedt zorg aan een diverse doelgroep, van kinderen tot ouderen, waarbij je professioneel omgaat met uiteenlopende gezondheidsvragen en emotioneel beladen situaties. Met jouw kennis en ervaring zorg je voor passende medische ondersteuning en werk je samen met andere disciplines om de beste zorg te bieden. Wat je gaat doen: - Uitvoeren van medische intakes: triage toepassen en zorgadvies geven op basis van de situatie. - Behandelen van klachten: medische vragen beantwoorden en waar mogelijk zelfstandig oplossen. - Diagnoses stellen: beoordelen of doorverwijzing naar een huisarts of specialist nodig is. - Coördineren van doorverwijzingen: afstemming met ketenpartners zoals GGZ, GGD en maatschappelijk werk. - Aanwezigheid op locatie: gedurende een aantal dagdelen per week zorg bieden en ondersteuning bieden aan cliënten.
