Verpleegkundige Revalidatie & Herstel Verpleging Gorinchem Oproepcontract 18-10-2026 Op de afdeling Revalidatie & Herstel verleen je zorg 'met je handen op je rug'. Niet letterlijk natuurlijk, maar je helpt cliënten stap voor stap op weg naar herstel en zelfstandigheid. Na een operatie, ongeval of ziekenhuisopname werken zij hier aan hun zelfredzaamheid, zodat ze weer naar huis kunnen. Wat ga je doen? Jouw focus ligt op herstel. Samen met de cliënt kijk je naar wat nodig is om weer naar huis te kunnen en werk je toe naar persoonlijke doelen. Je werkt voornamelijk avonden en nachten en speelt juist dan een belangrijke rol in het herstelproces. Er wordt doelgericht gewerkt aan de hand van zorgpaden. Je ondersteunt bij ADL-activiteiten en persoonlijke verzorging, maar stimuleert cliënten vooral om zoveel mogelijk zelf te doen. Jij begeleidt, coacht en oefent samen, altijd met oog voor wat iemand nog wél kan. Op de afdeling zie je veel verschillende ziektebeelden, waaronder CVA, electieve orthopedie en trauma. In een revalidatietraject van gemiddeld 3 tot 10 weken bouw je echt een band op met cliënten. En het mooiste? Je ziet mensen groeien. Soms gaan cliënten zelfs fitter en sterker naar huis dan vóór hun opname. Daarnaast: - verleen je verpleegkundige zorg en ondersteuning aan cliënten; - denk je mee over het zorgplan en rapporteer je over de voortgang; - geef je cliënten de juiste begeleiding en voorlichting; - ben je een verpleegkundig aanspreekpunt voor collega's; - voer je risicovolle en voorbehouden handelingen uit. Waar ga je werken? In het Gasthuis wordt kortdurende zorg geboden binnen Revalidatie & Herstel. De doelgroep is divers: van cliënten met electieve orthopedie en trauma tot oncologie, COPD, hartfalen en NAH, met specialisatie in CVA. De afdeling is volop in ontwikkeling en werkt toe naar een sterk therapeutisch klimaat, waarin cliënt en zorgprofessional optimaal samenwerken aan herstel en terugkeer naar ...
