Kom jij het thuiszorgteam versterken en de cliënten in de omgeving SED (Stede Broec, Enkhuizen en Drechterland) de aandacht geven die ze verdienen? Wat ga je doen Als Verpleegkundige stem je de ondersteuning af op de behoefte van de individuele persoon, je denkt graag vanuit mogelijkheden. Als verpleegkundige ondersteun en coach je het team, verricht voorbehouden verpleegtechnische handelingen en ondersteun je de cliënt bij het wonen en helpt met praktische en emotionele problemen. Daarnaast stel je samen met het zorgteam het zorgleefplan van de cliënten op. Je werkt methodisch, signaleert veranderingen, rapporteert en evalueert volgens dit plan. Voor je collega's ben je een aanspreekpunt, ondersteunt hen in het oppakken van de taken en verantwoordelijkheden en stimuleert de samenwerking. Daarnaast bouw je actief aan de (professionele) relatie tussen jou, jouw cliënten en eventueel de mantelzorger. Werken in de wijk (thuiszorg) betekent direct contact met onze ouderen. Zo krijg je een goed inzicht in de behoeften en wensen waardoor je gericht aan het werk kan. Het persoonlijke contact zorgt voor vertrouwen en een sterke relatie tussen jou en de client. Jij maakt daadwerkelijk het verschil en deze impact zie je meteen terug in je werk. Het werken is afwisselend en dynamisch; geen dag is hetzelfde doordat je te maken hebt met verschillende cliënten, situaties en uitdagingen. De lijnen zijn om samen met de wijkverpleegkundige en je collega's te kunnen overleggen. Natuurlijk samen bespreken we jouw wensen graag bij een kop koffie of thee om te kijken of we een match kunnen maken. Een keer meelopen? Ook dat kan! Wat bieden wij Salaris in FWG 45 CAO VVT; Voor het uitvoeren van de werkzaamheden in de wijk stellen wij een werkauto ter beschikking. Zo kun je comfortabel en zelfstandig naar onze cliënten reizen; Opleidings- en ontwikkelingsmogelijkheden; Reiskostenvergoeding, 8% vakantiegeld, maar ook een eindejaarsuitkering van 8,33%; Faciliteiten om ‘goed in je vel’ te
