Waar kom je terecht?

Cadenza is de cluster voor verslaving en dubbeldiagnose, met vijf gespecialiseerde afdelingen. Je vaste standplaats voor deze functie is Cadenza 5 maar je kan op alle Cadenza afdelingen ingezet worden.

Cadenza 5 biedt intensieve dagbehandeling voor mensen met verslavingsproblemen, vaak in combinatie met andere psychiatrische aandoeningen. De behandeling start met een kort programma gericht op stabilisatie en nuchterheid. Nadien kunnen cliënten aansluiten bij een behandelgroep met focus op Acceptance and Commitment Therapy (ACT) of schematherapie.

Jouw functie

Als begeleider patiëntenvervoer binnen cluster Cadenza ben je verantwoordelijk voor het organiseren en coördineren van de begeleiding van kwetsbare patiënten naar consultaties buiten de campus van Boechout.

Concreet sta je in voor:

  • Je plant en organiseert begeleidingen naar externe consultaties, onderzoeken en zorgafspraken.
  • Je staat in voor het administratief aanvragen en opvolgen van consultaties, in nauwe samenwerking met de zorgafdelingen en behandelende artsen.
  • Je begeleidt en ondersteunt patiënten tijdens hun bezoek aan externe zorgverleners, zoals een tandarts, polikliniek of ziekenhuis.
  • Je zoekt actief naar ondersteuningsmogelijkheden binnen het netwerk van de patiënt om begeleidingen mee mogelijk te maken. Hierbij werk je steeds in overleg met het behandelteam en de verantwoordelijke arts, zodat de ondersteuning maximaal aansluit bij de noden van de patiënt.

Als verpleegkundige binnen de mobiele equipe van het cluster Cadenza draag je daarnaast bij aan kwalitatieve en continue zorg op onze afdelingen. Wanneer je niet wordt ingezet voor patiëntenvervoer, ondersteun je één van de vijf zorgafdelingen binnen het cluster.

Je taken omvatten onder meer:

  • Je volgt patiënten op en begeleidt hen, met aandacht voor hun welzijn, herstel en continuïteit van zorg.
  • Je werkt nauw samen binnen een interdisciplinair team, waarbij verschillende expertises elkaar versterken om de best mogelijke zorg te bieden.
  • Je ondersteunt flexibel de zorgafdelingen volgens de noden van het cluster.
  • Je werkt voornamelijk in vroege en dagdiensten op weekdagen, met Cadenza 5 als vaste standplaats.
  • Je kan op vraag van de coördinatoren tijdelijk worden ingezet op andere zorgafdelingen binnen het cluster. Afhankelijk van je beschikbaarheid kan dit in vroege, dag- of avonddiensten, zowel tijdens de week als in het weekend.
  • Een diploma verpleegkunde op zak (bachelor, graduaat/HBO of basisverpleegkunde) met enige ervaring of pas afgestudeerd? Hoe dan ook word je warm onthaald en gecoacht door een professioneel team.
  • Heb je reeds ervaring of stage gelopen binnen de geestelijke gezondheidszorg en/of verslavingszorg? Dan is dit mooi meegenomen.
  • Je bent geboeid door de verslavingszorg en hebt veel zin om hiermee aan de slag te gaan.
  • Je beschikt over een goede dosis zelfstandigheid.
  • Je beschikt over een rijbewijs B
  • Met je enthousiasme, openheid en flexibiliteit onderhoud je en bouw je mee aan een toffe teamdynamiek.
