In de wijkverpleging help je cliënten om zo lang mogelijk zelfstandig en prettig thuis te blijven wonen. Dankzij jouw zorg, aandacht en betrokkenheid kunnen zij blijven leven op de manier die bij hen past. Als verpleegkundige bied je zorg aan cliënten volgens het individuele zorgplan. Je werkt met een diverse doelgroep en krijgt te maken met zowel planbare als complexe zorgvragen. Dankzij jouw klinische blik en verpleegkundige expertise signaleer je veranderingen in de gezondheidssituatie van cliënten en onderneem je waar nodig actie. Je voert verpleegtechnische handelingen uit, zoals wond- en katheterzorg, medicatietoediening en het instellen en bewaken van infuuspompen, bijvoorbeeld bij pijnbestrijding of palliatieve zorg. Daarnaast draag je actief bij aan de kwaliteit van zorg door verbeterkansen te signaleren en mee te denken over passende oplossingen. Samen met je collega's zet je je in om cliënten zo lang mogelijk zelfstandig thuis te laten wonen. Ook begeleid en ondersteun je collega's en leerlingen in hun ontwikkeling binnen het vak. Waar ga je werken? Je werkt in het team van de wijkverpleging Hattem en Heerde. Ons team bestaat uit collega VIG-ers en (wijk)verpleegkundigen. Je werkt zelfstandig op je route rondom de aanleunwoningen. We hebben binnen ons team korte lijnen en als het nodig is kun je terugvallen op een ervaren collega. Wij zijn een stabiel team en staan voor elkaar klaar! De diensten in Hattem zijn van 7.00 tot 15.00 uur en van 16.30 tot 23.00 uur. Daarnaast wordt er in Hattem in de middag gewerkt met bereikbaarheidsdiensten. In Heerde zijn de diensten van 7.00 tot 14.00 uur en van 17:00 tot 23.00 uur. - je beschikt over een diploma Verpleegk...
