Dan past werken in de wijkverpleging goed bij jou! Voor onze wijkteams in Assen en Aa en Hunze zoeken wij een Verzorgende IG . In deze functie ondersteun je cliënten in hun eigen vertrouwde omgeving en ben je tijdens een zorgmoment volledig gericht op de cliënt die voor je staat. Je bouwt een band op met cliënten, signaleert veranderingen in hun gezondheid en stimuleert hen om zo zelfstandig mogelijk te blijven functioneren. Werken in de wijk betekent afwisseling, verantwoordelijkheid en ruimte voor eigen regie. Je komt bij verschillende cliënten thuis en hebt een belangrijke rol in hun dagelijks leven. Geen dag is hetzelfde: iedere route brengt nieuwe situaties, uitdagingen en waardevolle contactmomenten met zich mee. Wat ga je doen? Je start je dienst vanuit het wijkkantoor en gaat vervolgens op route. Je ondersteunt cliënten bij persoonlijke verzorging, verpleegtechnische handelingen en medicatie. Tegelijkertijd houd je veranderingen in gezondheid en welzijn scherp in de gaten. Omdat je cliënten regelmatig ziet, speel je een belangrijke rol in het signaleren van veranderingen en het behouden van hun zelfstandigheid. Je werkzaamheden bestaan onder andere uit: - Verlenen van verzorging en begeleiding volgens het zorgplan; - Signaleren van veranderingen en hierop adequaat handelen; - Stimuleren van zelfredzaamheid (reablement); - Samenwerken met collega's, mantelzorgers en andere disciplines; - Werken met digitale systemen, zoals ONS van Nedap; - Inzetten van zorgtechnologie, zoals beeldzorg en medicijndispensers.
