En vind je het belangrijk om verder te kijken dan de zorg alleen? Dan voel je je bij team Heumen of team Malden vast thuis! Voorheen vormden we samen één team voor langdurige zorg thuis in de regio Heumen en Malden. Inmiddels zijn we opgesplitst in twee kleinere teams: team Heumen en team Malden. Ieder team heeft een eigen werkgebied, maar onze gezamenlijke opdracht blijft hetzelfde: mensen met dementie helpen om zo prettig mogelijk thuis te blijven wonen. De behoefte aan langdurige zorg thuis groeit. Vanuit onze reguliere wijkteams wachten al cliënten op een plekje bij een langdurig zorgteam. Met de uitbreiding van onze teams willen we meer mensen passende ondersteuning bieden. En daar kunnen we jouw hulp goed bij gebruiken! Zorg die verder gaat dan de voordeur Je leert cliënten en hun naasten goed kennen en ontdekt wat iemand nodig heeft om de dag prettig door te komen. Dat kan persoonlijke verzorging zijn, maar ook een kop koffie, een wandeling of samen boodschappen doen. Je sluit aan bij de belevingswereld van mensen met dementie en kijkt steeds naar wat wél kan. Samen kijken naar wat wél kan Onze teams zijn klein en bestaan uit collega's met verschillende leeftijden en ervaringen. Of je nu veel ervaring meebrengt of juist een frisse blik: jouw inbreng telt! Ook over je werktijden denken we graag mee. We willen nieuwe routes ontwikkelen die passen bij verschillende situaties. Denk aan een dienst van 09.00 tot 16.00 uur, van 16.00 tot 21.00 uur of een Helpende Plus-route van 13.00 tot 21.00 uur.
